Foundation Model

The Foundation Model of healthcare developed by Lambdanets and IHP uses a Health Information Exchange (HIE) system, a scalable system from a few practices to larger communities as counties or states.

Our data integration technology uses a centralized patient index that pulls information from different EMR/EHR systems at the state or community level avoiding thereby redundant and possibly inconsistent patient records.Doctors can use mobile technologies in their practices to access patient’s data, laboratory results, enter patient encounter information, and write prescriptions. Patients can access their own healthcare data from a patient portal on their computer or mobile device.

By integrating the data flow among different care facilities (hospitals, skilled nursing, assisted living, nursing homes and homes) and effectively managing the transition from the hospital to the patient’s home we aim at facilitating sustained home care, self-management, while still making it possible for effective intervention and care management. This is achieved at the community level by establishinga Provider Care Coordination center, where managers are in charge ofcare coordination plans of high risk patient groups.

High risk patients are identified using the Health Information Exchange technology and looking at the latest snapshot of patient data from EHR/EMR and other systems within the HIE. A screening software tool is used to identify patients with complications, such as diabetes, the frail elderly or other high risk patients.

Some of the benefits of the Provider Care Coordination are:

  • High risk patients are monitored more frequently by the care coordinator allowing doctors or care practitioners to identify possible complications faster and use preventive treatments, leading to a better care.
  • The comprehensive view of patients reduces the need to repeat certain procedures such asmaking diagnostic exams or taking blood samples.
  • Care plans have immediate impact on patients by improving their health.
  • In case of emergency the doctors have the patient’s History and Physical information readily available.
